Florida Keys Primary Care & Wellness is a physician-led primary care, wellness and aesthetic practice located at 29980 Overseas Hwy in Big Pine Key, FL. The practice was founded forty years ago and was known for most of that history as Big Pine Medical — a name that long-time Lower Keys residents will recognize. Today, that same practice operates as Florida Keys Primary Care & Wellness, with the same trusted location and an expanded scope of care.
What changed is not who we are. What changed is the kind of care patients in the Middle and Lower Keys are now seeking — modern hormone replacement therapy, medical weight loss with semaglutide, advanced laser and aesthetic devices, regenerative medicine, pelvic floor and cognitive wellness technology — services that, until recently, required a long drive to Miami. The new name reflects the wider scope. The forty-year community medical practice is unchanged.
Today our team of nine professionals serves patients ages 2 and up across Big Pine Key, Marathon, Summerland Key, Cudjoe Key, Sugarloaf Key, Ramrod Key, Bahia Honda, No Name Key, Little Torch Key and as far south as Key West.
Our mission is simple: to serve our community and patients across all their primary care, wellness and aesthetic needs — without forcing them to navigate three different clinics and three separate medical records. Whether a patient comes in for an annual physical, a hormone consultation, a same-day sick visit, a regenerative treatment or an aesthetic concern, they are seen by the same physician-led team that knows their full medical picture.

Tommy Iglesias helps lead the practice alongside and under the direct medical supervision of Dr. Gregory Keifer. While not practicing as a physician, Tommy focuses on practice operations, growth, and the integration of advanced aesthetic and wellness technologies.

From Marathon to Key West
Florida Keys Primary Care & Wellness serves residents and visitors across the Middle and Lower Keys. Our Big Pine Key office sits roughly halfway between Marathon and Key West along Overseas Highway, making us easy to reach from Bahia Honda, No Name Key, Little Torch Key, Ramrod Key, Summerland Key, Cudjoe Key and Sugarloaf Key. Patients also drive in from Marathon to the north and from Key West and the surrounding lower-Keys communities to the south. The Lower Keys deserve a local medical home for both primary care AND modern wellness — and we built our practice to be that home.
